These are best done by your own doctor. The insurance company will give you and your doctor very specific instructions on what they require. The usual is a half hour appointment with paperwork completed by you beforehand and be prepared for a blood test, urine sample and full physical examination, not often involving rectal or vaginal examinations. The company will usually pay the fees if they have requested the medical.
